October Journal Club Article

Last month everyone read about Chemotherapy Induced Nausea and Vomiting, or CINV.  This month we will explore symptom management and interventions for CINV.

Please return your answers to Melissa Luebbe c/o Pain Management, or, by email to melissaluebbe@comcast.net

Please go to, "Links You Might Enjoy," in the upper right hand side of the blog.  Select http:// www. ons.org, next select Clinical Practice, and then CINV.  Please read or listen to the podcast for the first article titled, "Interventions."  This article is by Nancy Hawkins and Steven Greenburg.

Questions:

1)  Anticipatory Nausea/Vomiting, is it real or imagined?   What medication is
     recommended for this problem?

2)  Is it the Chemotherapy or the pre-medications that are causing
     the patient discomfort?

Discuss some of the potential side effects from  the following:
Corticosteroids, 5-HT3 receptor agonists, NK1 receptor antagonist,
     Metoclopramide, and diphenhydramine.

3)  Intranasal sprays and transdermal patches, (Sancuso Transdermal
is already available), are being investigated for antiemetic therapy.
Why do you think these routes could be affective against CINV?
In what type of setting could they be most beneficial to patients?

4)  Could someone please share on the blog what happens when
Dexamethasone is pushed too quickly as a pre-medication?
How fast should it be given IVP?
