January 2018, Medicinal Cannabis

There is an explosion of interest and information regarding the use of medicinal cannabis. As more states pass laws that legalize the medicinal and recreational use of cannabis,  oncology nurses can be positioned as knowledgeable educators and advocates for our patients.

An important point to remember is that cannabis and most cannabinoids are federally illegal. The ability for our patients to apply for medical cannabis cards is totally dependent  on the state that they reside in. Doctors Abed and Raed Rahman can sign the medical cannabis application to authorize that a patient meets criteria for medical cannabis, the patient must be a documented resident of Illinois. They are not prescribers and are not involved in dosing. 


Our first article Medicinal Cannabis: A Nurses Primer does a great job explaining why we need to de-stigmatize cannabis and view it as a medication with palliative and potentially healing properties. Understanding Medicinal Cannabis in Cancer Care summarizes medicinal cannabis basics and touches on current research with solid tumors. I was fortunate to attend ONS Congress last May and hear Carey Clark's presentation on medical cannabis. Carey is a serious cannabis scholar, nurse educator and patient advocate. The second article is loaded with many links which will bring you up to speed on everything from state laws governing cannabis to the American Cannabis Nurses Association standards of practice.

  • What role can medicinal cannabis play in the current opioid / heroin crisis? What fatal side effect can occur with opioids and heroin that has never been documented with medicinal cannabis?

  • Give an explanation of the delta-9-tetrahydrocannabinol (THC) and Cannabidiol (CBD). What effect does THC and CBD have on the endogenous cannabinoid system?
  • Soft ball question - You have a patient who has a medicinal cannabis card issued to them from the state of Colorado where they reside. Is it legal for them to bring their legally prescribed medicinal cannabis with them when they come to MRMC for treatment?      
Discussion Question 

Last year I posted a list of possible nursing goals and resolutions for 2017. For those of you who participated in Journal Club last year, type January 2017 into the search box and see if you met your goal - sharing is optional. I have brought the list back for 2018, select one of the nursing resolutions and share with everyone why it speaks to you. Of course, everyone is free to add any goal not listed.


Nursing Resolutions and Goals 2018
  • Make a difference to someone  else's nursing career and mentor a new nurse. 
  • Seek out someone that could become a mentor to you.
  • Foster more collaboration and communication between physicians and patients.
  • Give yourself permission to say "no" at work or home when you feel overwhelmed.
  • Stop nurse bullying when you see it, eliminate gossip and back biting by not participating or listening to it.
  • Dust off your transcripts and go after your BSN or an advanced degree!
  • Find something positive to say to a co-worker you are not fond of.
  • Actually take the 30 minutes that you are given each day to eat, go for a walk, or relax.
  • Study for your OCN or become certified in your specialty area.
  • Develop some healthy ways to cope with on the job stress - any suggestions?
  • Avoid unhealthy stress eating and keep some healthy snacks at your work station.
  • Write down a list of reasons that you love your current job. If after ten minutes you are still staring at a blank piece of paper, talk with your manager or update your resume.

  1. Re-looking at my goals, I would like to continue to included more active activities into my life or as others would say "exercise." As my kids are getting more self-sufficient, I feel like I finally have more free time and want to participate in life outside of them. I am in the brainstorming phase and not sure of how I will pursue this goal, just yet. Work in progress.

    1. As my kids became older, I felt like I let friendships disappear. No time due to work, home, family. So started going for walks with friends as they started having more free time also. This satisfies girl time and walking time. Also I read fiction novels while on exercise bike and stepper machine. Satisfies fun reading and exercising. Tried the treadmill and reading... don't recommend it.

  2. Re-looking at my goals I completed the investigation process in pursing further education but thinking of pursuing another certification vs. my masters. As far as this year goes, I am still trying to become healthier. I am focusing on drinking more water at work. For me, I find this to be a struggle. Here's to a happy and healthy 2018!!!!

      I happen to know you are also playing a lot of tennis. I spoke with a wonderful dietician who suggested drinking an ounce of water after every bite of food for a week, then your body will crave water making it very easy to increase your intake.

  4. Re-assessing goals for 2018: continue preparation for the OCN-have spent a small fortune on books now wish I could buy the time in which to read them:-)...Like most, fitness is always a goal that challenges me to attain:-(

  5. Professional goal for 2017 - Met! I passed my OCN exam. Professional goal for 2018 that speaks to me; dust off your transcript and go after an advanced degree. I've had some set backs in the past which hindered my education. This year I feel that I am in a good position to re-visit the options for an advanced degree and choose one that works for me. I would like to challenge myself to complete a minimum of 2 courses in 2018 towards an advanced degree.

  6. 2017 goal: "My goal is to be a productive complainer! Not just complain about an issue but to have a valid solution to rectify that complaint. We often fall into a cycle where everyone can complain about something but no solutions are discussed. " I think I did pretty good with this. While I still find myself complaining (not me!) I am still working toward making it productive. I have formed a work group to facilitate the flow between CNCM and remotes so it's not just complaining about who does what but working through it to have more role delineation. This year my goal is to achieve my OCN.

  7. I have chosen mentoring a future nurse. My daughter is in college with a nursing major. So I try to impart positive stories both current and past in my career. (Respecting confidentiality of course.) I try to motivate her with interesting examples on the differences we make in patient's lives. I emphasize the respect and trust people place in our abilities and our character. I want her to approach a nurse career with great anticipation. So I am starting now to mentor my daughter and introduce her to our world. Humor also adds to her accepting my advice, as our children often tune us out. So for years to come, I will mentor her, teach her how to cope with career stress, and encourage her. She is the next generation of nurses, we have to nurture them and excite them as they take our places.

  8. Hello everyone,

    My goal last year was to take classes toward my BSN. This year, I hope to do the same. I completed 3 classes last year and hope to do 3-4 more in 2018. I would also like to become OCN certified. The idea of it is stressful and scary but I KNOW I will be much happier and be proud once I have done it! Good luck to everyone achieving their goals in 2018!

      Great goals, I am doing the same as you are 3-4 classes a year until I have my BSN. I think the best way to work toward your OCN is schedule a date then you will forced to find the study time. Maybe plan to take classes when you are not in a class so you can study.

      Best of luck on completing your BSN. I obtained mine last June after funcioning as an ADN for about 8 years. I was an arduous task that posed many challenges for my time management. However, I made it through and it provides me with great pride and a new goal of pursing a masters degree. I know you can and will do it.

I was successful in obtaining one of my goals from 2017, which was to obtain my CBCN. :) I also wanted to take a class focused on Spanish for medical professionals but I was unable to take the class due to scheduling conflicts. I would like to aim to take the class this year and in addition to take better care of myself in terms of not holding onto insignificant issues or past frustrating circumstances that are out of my control; I even bought I a decorative sign that says "let it go" for my living room to remind me (and my husband) on a daily basis.

    1. Go Elizabeth! the Spanish class sounds exciting! I think its great that you're interested in learning medical terminology in Spanish. This is something I have been wanting to do also (for Spanish speakers of course)but then started the CBCN and now I have to focus on the OCN.   10. My main goal for 2018 is to take and pass the OCN exam! I have already taken the class but I need to find the "push" to actually start studying. Hopefully by the end of the fiscal year I will be certified. Another goal that I have is to continue to mentor new hires and/or new nurses to the clinic. I enjoy teaching and showing others the responsibilities we have in the clinic, how we function as a team and as an outpatient clinic. I had a great nurse mentor me when I started with the team; she was very kind, knowledgeable and had a lot of patience. This is something that I always keep in the back of my mind :)

    I have to admit, I am really torn with going back to school for my BSN. There are financial concerns, concerns if I have the time to devote to classes without taking away from time with my family and friends and can I still learn? I am setting a goal toward taking small steps to determine if this is what I want to do such as, taking my boards for holistic nursing, studying and taking boards to become OCN certified and maybe then I will know if going back to school for my BSN is right for me. In the meantime, I will work on being the best nurse I can and balance that with some "me time" to enjoy my life as we know, life is very short.

    This year’s goal:

    Wow, what a difference a year makes. I have become certified as a holistic nurse and I have also become OCN certified. I took the big leap and started the BSN program at Chamberlain School of Nursing. I hope to complete classes by August of 2019. This year my goal is to give myself permission to say “no”. Not only say “no” but to reach out for help before I become overwhelmed.

  12. My goal this year is that same as it was last year, to study for the OCN exam. Like Roxanna, I am havine some difficulty getting this process started as I have many hobbies outside of work that are equally important to me and I am also committed to excercizing regularly.

      I also had a difficult time getting the preparation process started. It feels like such a daunting task with such an enormous amount of new and confusing information. I just decided to buy the OCN secrets book and take the advice of the authors. When I took my NCLEX and CNOR exam, I found it helpful to take things in small portions so I don't get too overwhelmed with so much new information. Practice test questions also proved to be very helpful, including reading all of the rationale for the correct answers.

    3. Mary, I agree that finding time to study for the OCN while working full time is challenging. Maintaining a work life balance is important to ensuring you are coming to work refreshed each day. I found that studying for the OCN felt like I was taking work home with me. I chose to read the OCN book slowly over a few months so that I didn't feel overwhelmed.

    4. Mary, I also was having difficulty getting motivated to study. Signing up and picking a date was what got me motivated to start my studying. I than like Brittney did a little each day so I did not feel overwhelmed.

    My goal for 2018 is to take and pass the OCN exam. I have been studying a little bit each day for a couple of months. I plan to take the exam in March which is the first month that is available in 2018. I am still awaiting my authorization to test notification from OCN. The material seems overwhelming, but I think my strategy of doing a little bit at a time will keep the information fresh in my mind up until test time.

  14. My post from last year was "As we all know health care has changed and we are all feeling the effects of this here at MRMC. So, my goal is to try to continue to have a positive and thankful attitude in this changed environment!". This is definitely still true and will continue to be my focus for this year. It seems that once we get used to changes that are made, more changes happen. But I am still thankful to be doing a job that I love what I do, so I will continue to try to focus on the positives and let go of any negativity.

  15. My goal for 2018 is to prepare to take the OCN. I believe I am slotted to take it in 2019 but I just want to give myself ample time to study. Honestly, the exam itself is intimidating to me but I know with early preparation it will ease my nerves a bit

  16. Well my 2017 goal is ongoing, I did take an OCN prep course and applied to test however the allotted time was up for the year apparently and I now have to wait to take it until later this year. I do plan to continue to study aside from my current BSN courses I am in.
    I think taking 2018 to continue to work on these same personal goals for me would be attainable and fulfilling so I plan to do so. As hard as it can be sometimes, a little bit at a time. Eventually I will get there.

  17. Last year my goal was to take my holistic boards…and pass! I successfully completed those goals. I also mentioned that I wanted to use more of my holistic training in practice. I don’t feel like I have done that. This year I am going to try again! My goal is to take a class or ceu’s to refresh my memory and motivate me to incorporate my holistic training into my nursing practice.

      That is a great idea. I have found that by incorporating more self-care and holistic practices in my life, and seeing firsthand the positive outcomes, I am eager to share my experiences and my knowledge with patients. It almost comes out as second nature now when I am teaching them on other topics. Many of them have come back and have used what I have shared with them and they say it has helped them and they are very great full.

  18. My 2017 goal was to eat lunch. I partially meet this goal: I eat at my desk, while I continue working. As I mentioned last year, once the weather warms up I'd like to stroll once or twice around the block.

    One goal I have been AWESOME with in 2018 is finally exercising again! Since Jan 1st, my husband and I have worked out in the early morning, long before our 2 young boys get up. We only do 30min, but it's concentrated. This is a huge milestone for us, as we let is slide the past several years, due to our children. They are currently 2 and 3.5 and we decided that we just had to find time to fit this in again. :)

    1. Theresa - remember those days of young kids! I used to go to an exercise class that had babysitting and my husband used to lift weights inside the baby gate and leave the kids on the outside! We did what worked. Lost a lot of it when we were running to games...but proud to say that this is instilled in all 4 of our kids. I always had the guilty mother feeling if I left the kids because of work but now my kids come home from work and go to the Y. The kids do an activity or go into the play area. There is not as much stigma now -it is much more positive - for parents to go work out after a long day away from the kids. The kids do fine and this is now their routine. Good for you!

  19. My 2017 goal was to take better care of my body by exercising more. I've not been as consistent with my gym time this winter, but was able to complete the Hot Chocolate 15K in late October-yay!

    My 2018 goal will be to mentor a new nurse. I've already started on this by having a Marquette nursing student shadow me in the Internal Medicine Clinic for two days. She hopes to come back over Spring Break for additional time, and I hope to be able to provide some other experiences for her at CTCA. The mentoring role is a good fit for me, as I enjoy encouraging others in whatever they are pursuing, and also can be honest in my depiction of the highs/lows that come with working in the health care field.

  20. I am continuously working on self improvement and stress management. I have successfully decreased stress levels and feel calm and centered. I have been doing yoga for over 5 years, however, with more commitment during last year. I also read a lot of spiritual books, specifically about meditation and mindfulness.
    I also started meditating anywhere anytime. Short periods of focusing on breath and watching own thoughts do help. I remind myself that all we have is present moment. Our past is history, and our future is mystery. This approach helps to diminish anxiety.
    I also started writing positive thoughts, things I am grateful for now.
    I cook home meals and pay attention to what I eat. Although, being a nurse in surgery puts some limitations on how you plan your meals, I try to remain hydrated and eat healthy snacks whenever time allows.
    Overall, I put a lot of effort in creating healthy habits, daily self-reflection, staying positive, keeping clean both my body and mind.

  21. Develop some healthy ways to cope with on the job stress - any suggestions? I make a point to go work out at the park district - 2-3 times per week. If this does not happen I do it at home or weather permitting (I do not walk in the cold) my husband and I walk every night. I have found an exercise called "Classical Stretch" that I love. I might only do 10-15 minutes but it stretches you out and re-energizes you.

    1. I find meditation to be one of the best ways to cope with the stresses of our jobs Pat! I use an app called Calm, it's really great. A meditation I try to recall when things are really crazy is called "Mountains," and it centers around imagining yourself as the most beautiful mountain you've ever seen and thinking about how a mountain is generally unchanging as seasons and the world change around it. This ties into the craziness of life, as chaos erupts around me, I tell myself, "I am the mountain," as a way to remind myself that as long as I am okay and strong and unchanging amidst the chaos, then that temporary chaos cannot affect me negatively. It sounds crazy to say that it really was life changing.

    2. I agree with you katrina, meditation, especially early in the morning, before the chaos in your house starts. Realy help to calm yourself. Meditation can be done before your day starts and during or after. Anytime of the day. It relaxes your mind and soul. SELF CARE is mandatory for me.

  22. My goal for last year was to take more time for practicing self-care which I have done. I would like to continue with that goal. One of the things I can add to my self-care routine is to actually take my 30 minute lunch break to eat and relax/de-stress. My other goal is to finish my MSN this year...3.5 classes left!

  26. My goals last year were to eat healthy, exercise regularly and give myself time to rest and relax. For this year, I will continue to do the same, because I believe that being healthy and in good spirit helps me a lot as a nurse, as a father & husband and all my patients as well. But on top of my previous years goals, I'm challenging myself to take & pass any State approve certifications related to my field. ( fingers-crossed )

  27. I was happy to see I met my goal last year and even surprised myself and obtained my BSN in record time... making it my 2nd Bachelor’s Degree. This year I really need to take the 30 minutes away and relax, eat, etc... that is my goal for 2018 —> taking the “me” time I deserve, even at work :)

  28. I achieved my goal last year of being CEN certified. this year I also started on doing more self care activities such as daily 20-30 minute exercise if my schedule allows and trying to eat healthy.

  29. Unfortunately I did not meet my 2017 goal for obtaining my OCN. I studied but never ended up taking the test. I do plan to continue to study this year and hopefully take the test.

    I also hope to take better care of myself, both mentally and physically.

  30. My 2017 has been met which was: My 2017 goal is to choose to be happy at work. Happiness is largely a choice. So I will continue to think positively about work. I will avoid negative people, complainers, and gossip. I will find co workers who I choose to spend my time with.I will choose to only be happy at work. I think with our team transition in clinic, teamwork is excellent and positivity is shining!

  31. I did meet my goal for obtaining my OCN certification. My other goal was to improve communication but I feel that there is always room for improvement so I will continue with that goal. The new goal I will work on for this year is saying no both at work and in my personal life when I am feeling overwhelmed. I am not very good at doing that.

  32. My goal this year is to avoid unhealthy stress eating and keep some healthy snacks at my office. Stress can increased my appetite and starts eating junk foods. Just started on weight watchers 2 weeks ago, I've lost 4 lbs. SELF CARE is mandatory for me.

  33. My goal this year is to avoid unhealthy stress eating and keep some healthy snacks at my office. I have done well on the South Beach Diet this last year and would like to continue with Level 2 and 3. I have gained back some weight especially over the holidays and with some family issues. More than anything I need to include a steady diet of exercise as well as eating healthy. I would like to surround myself with like mind and receive as well as offer support to others with similar issues. So don't be afraid to ask how I'm doing and hold me to my goals. Thanks.

  34. My New Year’s resolution last year (2017) was to go back to school to obtain my master’s degree in nursing. I started school last February at Chamberlain College of Nursing in the Family Nurse Practitioner program. I now have 1 year completed and 1.5 years to go. My second goal last year was to obtain my oncology nursing OCN certification. I am proud to say I also met this goal and passed the OCN exam this past October. My New Year’s resolution for 2018 is to continue going to school and start my clinical rotations. I also rejoined the gym this year and plan on making more time for stress free activities.

  36. One of my goals last year was to obtain my Holistic Nursing certification. I did get my certification. The other goals were to drink more water and exercise. I am drinking more water and less soda but still not exercising. So for this year I am making exercise a firm goal as well as losing the last 20lbs.

  37. My goal last year was to start my BSN and I have done that. I started at Western Governors in December and successfully completed my first two classes.

  40. My Goal this year is to allow myself to say "No." Often times guilt gets the best of me and I end up giving in to whatever the request may be. While I am getting better at this, I constantly struggle with the remorse. I plan to let things go and simply going with the flow.

  41. I was able to meet my goal for 2017 along with my wonderful peers. I received my AHNA certification. Had started my BSN, but due to personal reasons was unable to continue. Maybe...I would like to complete my OCN certification for 2017 and this is my year of self-care and joy after a grueling past few years. The happiness is amazing. Thank you all who love me:)

  42. I was not involved in Journalphoria last year so my goals currently are:
    1) Pass the OCN test in April
    2) Continue to grow and learn in my new role as a Nurse Navigator
    3) Try not to mess up being the Chair of UBC
    4) Stop stuffing my face when I'm not hungry!!!!

  43. Im working towards a couple of goals for 2018. First, is to make a difference in someone else's nursing career and mentor new hires/new nurses. I enjoy teaching, showing, and being a resource esp in the infusion center. Secondly, I would like to get motivated to take the OCN, and pass it! Studying is the hard part for me, fitting it in my busy schedule. Gotta stay positive because I know I can do it!

  44. My goal was to have my 30 minutes break that are given to us to eat, go for a walk outside(during summer) or relax. We in stem cell care managers are being reminded by our secretary daily that it's 1200 and time for lunch break in our skype group. During this break, CM will not accept any incoming calls and secretary will take messages and email it to the group. This way care managers get to eat uninterrupted and can relax and not to be hurried in eating. This is also good for digestion. This also helps us to step away from the computer so as not to strain our eyes. Then after lunch break, we can take care of the messages that were emailed to us. Another goal is to keep some healthy snacks at work station. I try to keep nuts and fruits and veggies at my work station.

  45. My goal this year is to study for my OCN. I technically have another year before I HAVE to be certified, but i'm also going to school so i'd rather have it done as soon as possible before school and work gets too hectic and I become too overwhelmed. I've never taken the OCN exam before, and those I've talked to who have taken it has told me that it was not an easy test. Makes me a little nervous but it helps that I work in the infusion center. It also helps that we have a lot of resources available to us here in CTCA, I think this goal is attainable as long as I put effort. Wish me luck!

  46. For 2018, I'd really like to obtain my OCN certification. I had it for many years but let it lapse when I left oncology for a couple of years and it's time to earn that again. There have been so many changes in oncology treatments over the last few years and I know that studying for the test would add a lot to my knowledge base which in turn will help me take care of patients more effectively.
    My 2017 goal was to take my 30 minute lunch break every day and for the most part, I have met that goal. There have been a few times where it was super busy and difficult to get away but I try to make the break happen for my mental health:)

  48. My goal for this year was to achieve my BSN and be OCN certified. I am also on the completion of my BSN come after February 25, 2018. I am so blessed and thankful that CTCA has helped me in any way to further my education and continue to encourage me to go beyond what I have expected for myself. This year, I aim to achieve my OCN. I know that hardwork and perseverance will play a very important part of this journey and I am ready to shoot for it.

  50. I'm going to try one more time to post this because it seems to keep disappearing!

    My goals last year were to cope with stress and less stress eating. Unfortunately, stress eating is still one of my favorite things to do, so I will definitely be working on that! I cope with stress much better, but it's always a work in progress.

    This year I plan to regularly write a list of what I like and love about my job, as a way to check in and see how I'm feeling overall.

  51. The goal that speaks to me is for 2018 is: Give yourself permission to say "no" at work or home when you feel overwhelmed. I tend to say "yes" and even "volunteer" to do something, just because I think I should. At times, I end up getting mad at myself for saying "yes", especially when I realize I don't have the time or energy to do whatever I committed myself to do. I need to think before saying "yes". That way, I can be more sure to spend my time on the things that are really important to me. That will make me a more balanced, happier, calmer person. At least I hope so! :)

The Institute for Safe medication Practices gave recommendations for eliminating medication mistakes of the following; establishing a no interruption zone, ensuring a do-n disturb approach, providing staff education, determining the best time for necessary interruptions, creating checklist, managing mobile deice, making system improvement, managing alerts, alarms, and noise, and gathering supplies prior to prescribing, preparing, or administering medication. While all of these ideas seem that they make work out in some nursing environments they would not be effective in the pacu. Safer practices that I use in my area is a mini timeout. Mini time out includes rechecking patients name band, looking at order, checking allergies in the corner of the screen, reading label on medication, verbalizing time out loud and dose. Another practice that I use is knowing the sounds of my alarms, each alarm has a specific sound or warning knowing the most important ones helps you priorities your day.
    The no interruption zone in theory might work in some situations, but is not realalistic, passing medications is important but rarely the most important task, nursing is unique environment where the nurse needs to constantly prioritizes what must be done. One example that sticks out in my head was years ago a patient had angioedema. The patient’s lips were 5 times their normal size. Random consult doctor entered the room at the same time as I did. I stated the patient had been started on Lisinopril the day before. The doctor used the computer on wheels to order the epinephrine, I called the pharmacy for the correct strength and dose for the situation. Pulled it from the pyxis, and we were well on our way back to homeostasis. My point is for a nurse not to be interrupted while passing medications is possibly abandonment, if the nurse isn’t available who is? The bar code scanning system I believe has made a huge impact on reduction of medical errors. It was found that between 96-98 % of medications given by nurse’s in hospitals where scanned (Dublin, 2013). Food for thought, the pharmacist verifying the doctor’s orders, shound not be interrupted.
